Medical Records/Invoice Requests

All medical records and invoice requests must be submitted online using our secure request portal.
Paper requests are not accepted.

While completing the form, please note:

  • You must select one of the images provided when prompted
    (Failure to do so will result in an error and your request will not submit.)
  • Click Submit once all required fields are completed.

  • On the next screen, add any required documents using the paperclip icon.

After submitting your request, you will receive an email from HealthEMS.

⚠️ Important:
You must click the FIRST link in the email to confirm your account.
Requests will not be received unless this confirmation step is completed.
